UI 2.3 - Initial application for maternity benefit

• Pregnancy / Birth to be verified by a registered midwife / doctor / caregiver

UI 2.7 - Income declaration

• Details of the amount you received or will receive from your current employer whilst on maternity leave.

UI 2.8 - Application to pay benefits into bank account

• Details of your bank account to be completed by your bank.
